From your dashboard click the "settings" tab then click "discharge checklist".  

From here you can add or remove any forms you would like. When you select "add form" all of the forms you have added in your "forms manager" tab will show.  You can select and remove any of your custom forms you want.

To create a new custom form select "forms manager" from your navigation bar under the settings tab. From here you can edit, view, or remove any of your forms.
